| Details:Renaissance Behavioral Health Systems and Mental Health Resource Center is seeking a LPN/Licensed Practical Nurse for our Medical Case Manager position. This position is located in our Downtown office. The Medical Case Manager-LINK is responsible for ensuring quality competency-based medical/health education services to the homeless population. The Medical Case Manager-LINK is responsible for follow-through medical services, monitoring of medical problems with clients, and assisting clients with applying for entitlement benefits. Some of the other essential functions include but are not limited to: Clinical/Educational Services 1. Monitors vital signs and completes Waived Testing to include pregnancy testing, glucose monitoring and drug and alcohol screening and monitors the weight of individuals served. 2. Performs TB PPD screenings with appropriate referrals to the Health Department when necessary. 3. Provides first-aid care and dressing changes. 4. Learns as much as possible about the culture of individuals served and recognizes the influence his/her own background has on his/her understanding and response to cultural differences when interacting with individuals. 5. Provides referral information to clients for indigent medical care. 6. Provides education for clients regarding HIV/AIDS, STDs, communicable diseases, hygiene and diet, disease prevention and health awareness. 7. Maintains the medical clinic to include the organization of medication cabinets, samples, maintenance of PPE kits and first aid box, ordering of medical supplies, and the upkeep of all medical equipment. 8. Monitors samples/coupons; contacts pharmaceutical representatives when medication is needed. Coordinates transfer of sample medications with other locations (i.e. Eastpark/MHRC, 20th Street/MHCJ) to ensure maximum usage of samples. 9. Documents nursing care in records of individuals served in a thorough, professional, accurate and timely manner. 10. Responsible for picking up medication from pharmacy on a daily basis. Administration 11. Maintains an inventory of all medication samples, incoming and dispensed. 12. Assists in coordinating care with clients’ case managers. 13. Assists clients in completing paperwork necessary to obtain entitlement benefits. 14. Assists clients with referral process for Vocational Rehabilitation. 15. Accompanies case managers during street outreach. 16. Provides first aid assistance to clients on the streets as needed. 17. Maintains appropriate documentation on all services provided to clients as well as all outcomes. 18. Maintains and demonstrates optimal nursing practice through continuous professional growth and development, specifically age related and culturally sensitive to the population served.
